投标人需提供以下文件

*（1）投标人须按招标文件第一册第四章格式IV-9.资格证明文件格式的要求填写和提交有关资料。
*（2）投标产品原产地在关境外的投标人必须是制造商或其下属子公司。如为投标人为制造商下属子公司，则须由制造商总部提供其与制造商的关系证明文件的扫描件，关系证明文件必须由制造商签字盖章并提供有效联系人和联系方式，以便核实；
*（3）投标产品原产地在关境内的投标人必须是制造商（不接受代理商投标）。
*（4）投标人未被中国石油招标投标网(www.cnpcbidding.com)暂停或取消投标资格。（根据《中国石油天然气集团有限公司投标人失信行为管理办法》的规定，失信积分达到8分时，暂停失信行为人投标资格半年；失信积分达到9分时，暂停失信行为人投标资格一年；失信积分达到10分时，暂停失信行为人投标资格两年。暂停期满后恢复投标资格，有效期内的失信记分继续执行。失信积分超过10分或在暂停期间再次发生失信行为的，取消投标资格三年。）上述内容以评标委员会现场在对应网站的查询结果为准。
*（5）投标人未被纳入中石油集团公司供应商、承包商、服务商黑名单（集团公司供应商、承包商、服务商黑名单可在中国石油招标投标网/www.cnpcbidding.com查询）。上述内容以评标委员会现场在对应网站的查询结果为准。
*（6）距开标之日前一年内，投标人在中国石油物资有限公司组织的招标活动中对评标结果的无效投诉（包括中石油集团公司机电办处理结果为：不予受理、驳回、维持原评标结果）达到一次及以上的，投标将被否决。
*（7）投标人须提供开户银行在开标之日前三个月内开具的资信证明原件或该原件的复印件（由投标人代表签字或盖章）。
*（8）第八章中要求的其他合格的资格证书。
*(9) 投标方应提供 2021 年至今（以合同签订时间为准）应用于中国境内 EVA 或 LDPE 生产装置，连续稳定运行 1 年及以上点式红外可燃气体检 测器和开路红外可燃气体检测器的使用业绩，业绩合同数量不少于 1 个， 每个业绩合同中包含点式红外可燃气体检测器和开路红外可燃气体检测 器数量各不少于 10 套。业绩清单应至少包括：用户名称、检测器类型及 型号、检测介质、数量、产地、使用装置、联系人及电话。业绩合同中 提供的产品须与本次投标产品型号和技术指标一致，其中技术指标不得 低于本次招标数据表的要求。提供能够证明其销售情况和型号、技术参 数的合同复印件和技术协议关键页复印件，可删除商务报价信息，并由 投标方代表签字或盖章，投标方所提供的业绩必须真实、有效。。

(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.cnpcbidding.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.) (6) The bid shall be rejected if the bidder puts forward invalid complaint one or more times in the bidding activities organized by China Petroleum Materials Co., Ltd. within one year before the bid opening date (including complaint rejected, overruled or judged to maintain the original bid evaluation results by the mechanical and electrical office).
(7)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
(8) Other Qualification and Achievement requirements stipulated in Chapter VIII.
(9)The Bidder shall provide the application performance of point-type infrared combustible gas detector and open-circuit infrared combustible gas detector applied to EVA or LDPE production devices in China from 2021 to now (subject to the time of contract signing), which have been continuously and stably operated for one year or more, and the number of performance contracts shall not be less than one. Each performance contract includes no less than 10 sets of point-type infrared combustible gas detectors and open-circuit infrared combustible gas detectors. The performance list shall at least include: user name, detector type and model, detection medium, quantity, place of origin, device used, contact person and telephone number. The products provided in the performance contract must be consistent with the model and technical indicators of the bidding products, and the technical indicators shall not be lower than requirements of the bidding data sheet. Provide copies of the contract and key pages of the technical agreement that can prove its sales, model and technical parameters, delete the commercial quotation information, and sign or seal by the representative of the bidder. The performance provided by the bidder must be true and valid.
