投标人需提供以下文件

*（1）投标人须按招标文件第一册第四章格式IV-9.资格证明文件格式的要求填写和提交有关资料。
*（2）投标产品原产地在关境外的投标人必须是制造商或其下属子公司。如为投标人为制造商下属子公司，则须由制造商总部提供其与制造商的关系证明文件的扫描件，关系证明文件必须由制造商签字盖章并提供有效联系人和联系方式，以便核实；
*（3）投标产品原产地在关境内的投标人必须是制造商（不接受代理商投标）。
*（4）投标人未被中国石油招标投标网(www.cnpcbidding.com)暂停或取消投标资格。（根据《中国石油天然气集团有限公司投标人失信行为管理办法》的规定，失信积分达到8分时，暂停失信行为人投标资格半年；失信积分达到9分时，暂停失信行为人投标资格一年；失信积分达到10分时，暂停失信行为人投标资格两年。暂停期满后恢复投标资格，有效期内的失信记分继续执行。失信积分超过10分或在暂停期间再次发生失信行为的，取消投标资格三年。）上述内容以评标委员会现场在对应网站的查询结果为准。
*（5）投标人未被纳入中石油集团公司供应商、承包商、服务商黑名单（集团公司供应商、承包商、服务商黑名单可在中国石油招标投标网/www.cnpcbidding.com查询）。上述内容以评标委员会现场在对应网站的查询结果为准。
*（6）距开标之日前一年内，投标人在中国石油物资有限公司组织的招标活动中对评标结果的无效投诉（包括中石油集团公司机电办处理结果为：不予受理、驳回、维持原评标结果）达到一次及以上的，投标将被否决。
*（7）投标人须提供开户银行在开标之日前三个月内开具的资信证明原件或该原件的复印件（由投标人代表签字或盖章）。
*（8）第八章中要求的其他合格的资格证书。
*(9) 投标方应提供 2021 年至今（以合同签订时间为准）应用于中国境内 连续稳定运行 1 年及以上，检测介质为含乙烯或醋酸乙烯的烃类气体的 点式红外可燃气体检测器至少 200 台和开路红外可燃气体检测器至少 50 台的使用业绩。业绩清单（包括：用户名称、供货气体报警器型号、数 量、技术协议，技术协议需要体现设计院数据表，厂商选型文件、产地、 应用装置名称、用户联系方式、投用时间），业绩合同中提供的产品须 与本次投标产品一致，其中至少包含本次招标中每种型号，同等设计要 求的气体检测器，提供能够证明其销售情况的合同复印件和技术协议关 键页复印件，可删除商务报价信息，并由投标方代表签字或盖章，投标 方所提供的业绩必须真实、有效。

(2) The bidder whose product origin is outside the customs must be the manufacturer or its subcompany.
(3)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)
(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.cnpcbidding.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.) (6) The bid shall be rejected if the bidder puts forward invalid complaint one or more times in the bidding activities organized by China Petroleum Materials Co., Ltd. within one year before the bid opening date (including complaint rejected, overruled or judged to maintain the original bid evaluation results by the mechanical and electrical office).
(7)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
(8) Other Qualification and Achievement requirements stipulated in Chapter VIII.
(9)The Bidder shall provide the service performance of at least 200 point-type infrared combustible gas detectors and at least 50 open-circuit infrared combustible gas detectors with the detection medium of hydrocarbon gas containing ethylene or vinyl acetate, which have been continuously and stably operated in mainland of China for one year or more since 2021 (subject to the time of contract signing). Performance list (including user name, supply gas alarm model, quantity, technical agreement, technical agreement needs to reflect the design Institute data sheet, manufacturer selection document, place of origin, application device name, user contact information, commissioning time), the products provided in the performance contract must be consistent with the bidding products, including at least each model in this tender. For gas detectors with the same design requirements, copies of the contract and key pages of the technical agreement that can prove the sales situation shall be provided.
