投标人需提供以下文件
The following documents must be provided
*（1）投标人须按招标文件第一册第四章格式IV-9.资格证明文件格式的要求填写和提交有关资料。
*（2）投标产品原产地在关境外的投标人必须是制造商；或者是制造商直接授权的代理商（进口产品开标前未进入中国关境内的，投标人必须是在关境外注册的公司；进口产品开标前已进入中国关境内的，投标人必须是在关境内注册的公司），代理商投标需具备以下条件：
①必须提供制造商对本次投标的唯一正式授权书（授权书必须由制造商直接授权，不允许二级授权）；
②投标人需提供营业执照等注册登记的有效证件复印件；
③投标人依法登记注册时间距开标日满两年，且距开标日两年内，投标人须具有同类产品至少两个不同年份的销售业绩，需提供证明其销售业绩的合同或订单复印件（至少提供2份不同年度的合同或订单复印件）或用户证明，并由投标人代表签字或盖章。
*（3）投标产品原产地在关境内的投标人必须是制造商（不接受代理商投标）。
*（4）投标人未被中国石油招标投标网(www.cnpcbidding.com)暂停或取消投标资格。（根据《中国石油天然气集团有限公司投标人失信行为管理办法》的规定，失信积分达到8分时，暂停失信行为人投标资格半年；失信积分达到9分时，暂停失信行为人投标资格一年；失信积分达到10分时，暂停失信行为人投标资格两年。暂停期满后恢复投标资格，有效期内的失信记分继续执行。失信积分超过10分或在暂停期间再次发生失信行为的，取消投标资格三年。不当异议的失信扣分详见附件6-5《投标人失信扣分》。）上述内容以评标委员会现场在对应网站的查询结果为准。
*（5）投标人未被纳入中石油集团公司供应商、承包商、服务商黑名单（集团公司供应商、承包商、服务商黑名单可在中国石油招标投标网/www.cnpcbidding.com查询）。
（6）失信扣分。第八章商务评价条款中增加失信扣分，如投标人有失信分，失信扣分=失信权重×100（商务分）×失信分/10。失信权重不低于10%。失信分以评标委员会现场在对应网站(www.cnpcbidding.com)的查询结果为准。
*（7）投标人须提供开户银行在开标之日前三个月内开具的资信证明原件或该原件的复印件（由投标人代表签字或盖章）。
*（8）第八章中要求的其他合格的资格证书。
(1) All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
(2) The bidder whose product origin is outside the customs must be the manufacturer; Or the agent directly authorized by the manufacturer. The agent shall meet the following conditions for bidding:
① The bidder should provide a letter with original sole official authorized by Manufacturer for the bid；
② The Agent shall provide copies of valid registration documents such as business license;③ The bidder shall have been legally registered for a period of two years prior to the bid opening date.Within the two years preceding the bid opening date, the bidder shall possess at least two sales performance records of similar products from different calendar years. Copies of contracts or orders evidencing such sales performance (copies of no less than two contracts or orders from different years) or user certificates shall be provided, which shall be signed or sealed by the bidder’s authorized representative；
(3)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)
(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.cnpcbidding.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.) (5)Bidders are not included in CNPC Suppliers, contractors, service providers blacklist (Suppliers, contractors, service providers blacklist show at www.cnpcbidding.com)
(6)Deduction for dishonesty. Add a deduction for dishonesty in Chapter 8 Commercial Evaluation Clauses. If the bidder has a dishonesty score, the deduction for = dishonesty weight × 100 (commercial score) × dishonesty score / 10. The dishonesty weight shall not be less than 10%. The dishonesty score shall subject to the query results on the corresponding website (www.cnpcbidding.com) by the evaluation committee.
(7)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
(8) Other Qualification and Achievement requirements stipulated in Chapter VIII.
